The Skills Programme: Incident Investigation Team Member (SP-250906) is a QCTO-aligned programme that prepares learners to serve as competent members of a workplace incident and accident investigation team. The programme responds directly to South Africa's need for credible incident investigation at the operational level, and it equips learners to preserve and collect evidence properly, interview witnesses without contaminating their accounts, and produce findings that hold up when they are relied on.
The programme integrates theoretical knowledge with applied practical work across the full investigation cycle — from classifying an incident and reporting it, through the six steps of an investigation, evidence collection and witness interviewing, to root cause analysis, report writing and presentation. On completion, learners are able to distinguish a hazard from a risk and an incident from an accident, follow a structured investigation methodology, gather and document the four kinds of evidence, conduct interviews that collect rather than close down information, contribute to identifying root causes rather than stopping at immediate ones, and prepare a report that separates fact from opinion. The price excludes the FISA exam.
The purpose of this skills programme is to prepare a learner to participate effectively as a member of an occupational incident and accident investigation team. A team member collects evidence, interviews witnesses and records what they say accurately, contributes to the analysis and drafts parts of the report, all to the standard set by the team leader. The team member does not decide who was at fault, does not disclose what one witness said to another, and does not discuss the investigation outside the team. An investigation establishes what happened and why so that it does not happen again; it is not a disciplinary hearing, and the two processes are kept apart.
This programme is designed for people who are appointed to, or expect to be appointed to, an incident investigation team. It is well suited to:
Learners must hold an NQF Level 3 qualification and have at least six months' operational experience in an organisational context. The experience requirement matters: the programme assumes you already know what a workplace looks like, what a shift feels like and what a supervisor does. The practical module is delivered in a simulated or controlled training environment provided by the training provider, so no live incident is required. Recognition of Prior Learning is available in accordance with the QCTO RPL Policy and must be applied for before the programme begins.
